A Letter from Dr. Fadool
Dear patient,
It is with many happy memories and one heavy heart that I am writing to inform you of the closing of my practice, Elements of Care Ob/Gyn. I will continue working, but in a hospital setting, which will allow me to spend more time with my family and friends. It has been an honor to provide medical care to you and many others over the past 23 years of practice in the Surprise area.
To receive continued medical care, please start planning to place yourself under the care of another physician. I have included a list of Ob/Gyn providers in the area for your convenience.
The contents of your medical chart are confidential. A Medical Records Release form must be completed for your records to be transferred to another physician’s office. You may also use the same form to receive a personal copy of your records in paper or digital format for a $25.00 processing fee.
PLEASE NOTE:
After December 22, 2024, patient medical records will be sent to Desert River Solutions. They will be the custodian of all medical records for Elements of Care OBGYN until 2044. Starting on December 23, 2024, you may start requesting your medical records from Desert River Solutions. To request your medical records, click on the web address www.DesertRiverSolutions.com/
